Parent Directory Sign

A Parent Directory is now being compiled to enhance communication among parents and parents, and parents and staff. This directory will be a "hard copy directory" and will be distributed to each participating family.

This is a voluntary directory. You may choose to have your information in the directory or choose to opt out. Please fill out the portion of the form that applies. If you don't want to participate, go directly to the bottom of the form and simply add your name to the "No" section.

If there is any line that you don't want to fill out (such as email address or phone number), that's OK. Just fill out the information you want to share.
